Community based services
Our Community Based Services Program assists families involved with the Cuyahoga County Division of Children and Family Services who are in need of resources, advocacy, and support. The program offers support groups for relative, foster, and adoptive caregivers, hosts reunification visits for relative or biological family members, as well as offers services for youth aging out of foster care. Staff provide a family-friendly environment to all participants and encourage independence and personal growth. The program has expanded to provide services to Southwest Cleveland as well (see below).

southWest Community based services
Southwest Community Based services provides resources and supports to families in fourteen communities in Southwest Cuyahoga County. Services include case management, parenting support and assistance with basic needs. Currently serving: Berea (44017), Brecksville (44147), Brooklyn/Brooklyn Hts. (44144), Brook Park (44142), Independence (44131), Middleburgh Heights (44130), North Olmsted (44070), North Royalton (44133), Olmsted Falls/Township (44138), Parma (44129, 44134), Parma Heights (44130), Seven Hills (44131), and Strongsville (44136, 44149).

Fatherhood INITIATIVE
The Fatherhood program focuses on fathers at Willow, Mound, A.B. Hart, and Warner Road Girl’s Academy. The program offers case management, support groups, parent education sessions, advocacy for fathers, and father-child activities and events such as the annual Father Daughter dance.

R.I.S.E (Re-Integration services and empowerment)
The RISE program provides case management and resource referral for individuals transitioning from incarceration back into the community.

TRANSITION-IN-PLACE case management program
Once a family is transitioning out of shelter and entering EDEN’s Rapid Re-Housing Program, we provide intensive case management, home visits, and referrals to community programs and resources to ensure stable housing is maintained. Families are eligible only through referral from shelter.
